You may apply to alter the name in your birth registration through the Divisional Secretariat, using the Registrar General's declaration forms.
What is wrong
- Using a new name to avoid debts or a court case
- Giving false documents with the application
- Using different names on NIC and passport
What you can do
- Under 21: parent applies with the child's and parents' birth and marriage certificates
- Over 21: fill the declaration form and publish a newspaper notice
- Attach a Grama Niladhari letter and proof you use the new name
- Submit to the Divisional Secretariat and get the amended birth certificate
- Then update your NIC, passport, bank and land records
